小山田老人保健施設
1986年の老人保健法の改正により全国で７カ所のモデル施設として、厚生省（当時）より指定されました。
日常生活の中でお年寄りの今ある能力を使うことこそ最大の訓練と考え、アクティビティや調理、創作活動などを行い生活の活性化を図っています。その人の在宅復帰を目標に住み慣れた場所で安心して暮らせることを一番に考えております。
