障害者支援施設 小山田苑
18歳以上で障害支援区分4以上、若しくは50歳以上で区分3以上の身体障害をお持ちの方に対し、入浴、排せつ、食事の介護と日中の機能訓練や創作活動の機会を提供します。また、区分３以上の身体・知的障害をお持ちの方に対する生活介護（通所）や区分１以上の身体障害をお持ちの方への短期入所、身体・知的障害をお持ちの方へ計画相談支援も行っています。
